OFFICIAL RULES FOR

“COOK AND CONNECT” SWEEPSTAKES 

The Skin Deep Media Corp. d/b/a The Skin Deep

1. These rules, terms and conditions (“Rules”) apply to the “Cook & Connect” Sweepstakes (the “Sweepstakes”) featured by The Skin Deep Media Corp. d/b/a The Skin Deep (“The Skin Deep”) on its online website,  http://shop.theskindeep.com/contest/cook-and-connect (the “Website”).  The Skin Deep reserves the right to cancel or amend these Rules without prior notice.  Any changes will be posted on the Website. The Skin Deep maintains an office address at 42 West Street, Suite 226, Brooklyn, New York 11222.
   
   
2. Entry Period:  The entry period (“Entry Period”) for the Sweepstakes starts on Monday, September 21, 2020 at 12:01 pm EST (the “Start Date”) and Sunday, October 25, 2020 at 11:59 a.m. EST (“End Date”). 
   
   
3. Eligibility: The Sweepstakes is open to all residents of USA who have reached the age of eighteen (18) by the Start Date.  However, notwithstanding the foregoing, the following residents of the USA are not eligible to enter into the Sweepstakes : employees, agents and representatives of The Skin Deep, its subsidiaries, affiliates, affiliated companies, advertising or promotion agencies, the suppliers of material, prizes and services related to this Sweepstakes, and any other stakeholder directly related to the holding of the Sweepstakes, as well as the members of their immediate family (father, mother, brothers, sisters, children and legal or de facto spouse) and anyone with whom these employees, representatives and agents reside.  The Sweepstakes is void where prohibited.
   
   
4. Entry Procedure:
   
   a) Main Method of Entry: To enter the Sweepstakes, each Entrant (“Entrant”) must fill out the Sweepstakes application form available at www.shop.theskindeep.com/cookandconnect and provide the following required information: (i) first name; (ii) last name; (iii) email address; (iv) phone number; (v) date of birth (to confirm that he/she has reached the age of 18).  In addition, each Entrant must: (vi) affirm that they have read and understood the  Rules; (vii) agree to receive marketing emails from The Skin Deep, and (viii) take a video or photo of you playing {THE AND} during a trip and post same on social media (and provide evidence of same either by tagging “@the_skindeep” on Instagram or emailing proof to contact@theskindeep.com), before the End Date.
   
   b) Alternative Method of Entry: Individuals who prefer not to submit entry forms online and/or do not want to be placed on The Skin Deep’s marketing distribution lists may enter by sending a completed entry form by first or second class post, to the following address: The Skin Deep, 42 West Street, Suite 226, Brooklyn, New York 11222.  Completed entry forms which are mailed (“Mail-In Entries”) must be postmarked during the Entry Period.  Any Mail-In Entries will not be deemed incomplete if they do not contain an email address so long as all other required fields are complete and a phone number is set forth so that The Skin Deep can contact the Entrant if he or she wins.  Entrants using this alternative method of entry shall not be placed on The Skin Deep’s distribution lists, but may be contacted through email for purposes of notifying such Entrant if he or she wins the Sweepstakes.
   
   c) There is no purchase requirement to enter the Sweepstakes. Purchases from The Skin Deep do not improve the chances of winning.  There is no charge to register with the Website if Entrants enter the Sweepstakes online. Entrants are responsible for the cost of their connection to the Sweepstakes web page via the internet. The Skin Deep does not guarantee continuous, uninterrupted or secure access to the Website. No Entrant may submit more than one entry.  Any attempt or suspected attempt to circumvent these limitations by using multiple email addresses or pen names, or any use of robotic, automatic, programmed or any entry methods not authorized by these rules, shall be deemed as tampering and will void all of the entries for such Entrants. All entries that are incomplete, illegible, damaged, contain an erroneous phone and/or email address or do not conform to or satisfy any condition of the Rules may be disqualified by The Skin Deep.
   
   
5. PRIZE
   
   Each of the three winners shall receive the Prize, which consists of : 1) an AIRBNB gift card valued at $600 USD; 2) an UberEATS or Blue Apron gift card (at winner’s choosing) valued at $200; and 3) any three card decks of winner’s choosing offered by The Skin Deep on the Website, (valued in the aggregate at approximately $75). The Prize offered must be accepted as awarded and is not exchangeable, refundable, or transferable.  No cash alternative is available for the Prize. In the unlikely event the Prize should become unavailable or irredeemable, The Skin Deep reserves the right to substitute an alternate prize which is reasonably determined as being of equal value. In the event the Prize is not accepted in accordance with its attached conditions, the Prize will be deemed forfeited. The Skin Deep is not responsible for lost or stolen gift cards.
   
   
6. DRAW
   
   Three entrants shall be selected randomly from all eligible entries received (or in the case of Mail-in Entries, postmarked) during the Entry Period. The draw shall be held on Monday, November 2, 2020 at 4:00p.m., EST, to provide sufficient time for receipt of Mail-in Entries.  The draw shall be held in the offices of The Skin Deep.  All Entries will be listed and numbered on a spreadsheet, including Mail-In Entries, and a random number will be generated through an independent computer program, using a blind procedure, which will reveal the selected entries.
   
   
7. PRIZE CLAIM
   
   In order to be declared one of the three (3) winners, the selected Entrants must:
   
   a) Fill in the entry form and complete and answer all the compulsory fields.
   
   b) Be reached by telephone or email by the The Skin Deep within forty-eight (48) hours after the draw. If The Skin Deep is unable to reach the selected Entrant by telephone or email within forty-eight (48) hours after the draw, all rights to the Prize will be forfeited and another entry will be randomly drawn. If The Skin Deep is unable to reach the second selected Entrant by telephone or email within forty-eight (48) hours of the draw of the second entry, all rights to the Prize will be forfeited and another entry will be drawn. If The Skin Deep is unable to reach the third selected Entrant by telephone or email within forty-eight (48) hours of the draw of the third entry, the prize will be forfeited;
   
   c) Complete and sign the declaration and waiver form (the “Declaration Form”) which will be transmitted by The Skin Deep and must be returned to The Skin Deep within three (3) business days of receipt.
   
   The three (3) winners will be named within thirty (30) days following the draw date for the Prize. In default of observing any of the conditions mentioned above or any other conditions stipulated in these Rules, the selected Entrant will be disqualified and the Prize that would have been awarded will be forfeited. Within the days following receipt of the duly completed Declaration Form, The Skin Deep of the Sweepstakes will contact the winner to inform him/her of how to claim the Prize.
   
   
8. PERSONAL INFORMATION: From time to time, The Skin Deep, may use the personal information provided in the entry form for the purpose of:

9.1. Each Winner, without receiving additional compensation, authorizes The Skin Deep to use their name, address (city, province), voice, statements, comments, photographs and/or other likeness, for any purpose whatsoever through all media, including, without limitation, the internet for advertising or promotional purposes, and they agree to sign any additional document that could serve to assign the rights to their image, in particular, to The Skin Deep.

9.2. This Sweepstakes is subject to the federal, provincial and municipal laws, regulations and by-laws.

9.3. Any forms that are incomplete, untimely received or postmarked, lost, irregular in any way or sent by an unauthorized method shall be deemed null and void and will not be accepted for the draws, and The Skin Deep assumes no responsibility therefor.

9.4. The Skin Deep will assume no responsibility for any problem, failure or technical malfunction of a network line, online computer system, peripheral telephone, software, server, provider, email or browser or any other technical malfunction that may occur, including, but not limited to, a problem of the transmission or non-transmission of an entry form, for any reason whatsoever. The Skin Deep is not responsible in any way for erroneous or inaccurate information, whether caused by website users or by the equipment and/or programs associated with or used by the Sweepstakes or by a technical or human error that may have occurred in conducting the Sweepstakes, including in the processing of entry forms. The Skin Deep assumes no responsibility for any error, omission, interruption, loss or fault relating to operation or transmission, failure of communication lines, loss or destruction or prohibited access to or modification of the entry forms. The  Skin Deep is not responsible for any injuries or damages that may result from the holding of the Sweepstakes. All entry forms become the property of The Skin Deep and will not be returned to the Entrants. The Skin Deep reserves the right to destroy all the documents pertaining to the Sweepstakes once the retention period as required by law has expired. The Skin Deep shall have no liability to previously selected entrants who have not timely responded or whom could not be contacted.

9.5 Winners agree to provide any information required by The Skin Deep to fulfill the Prize as per applicable laws, such as the Winner’s social security number when and if required for tax regulation compliance purposes. In addition, the Winners may be required to complete a general release and an affidavit of eligibility, and provide proof of age and state of residence.
